Information you provide

The enquiry form asks for your name, email address, preferred city in China, approximate travel dates, package interest, preferred language, and consent to contact and sharing. We also keep the messages exchanged about your enquiry.

Please do not send symptoms, medical records, identity documents, or payment details through this form or by ordinary email. This website does not offer online report storage or access.

How we use it

We use your details to respond to your enquiry, check provider availability and package scope, discuss the price and practical arrangements, and coordinate a visit if you decide to proceed. Sending the form does not confirm an appointment.

Who receives it

Formspree processes the form submission and delivers it to our email account hosted by Google. We may share the enquiry details needed to check availability and arrange a visit with a suitable checkup provider in China. That provider may keep its own copy under its own privacy practices.

Because you may be outside China and these services operate internationally, your enquiry details may be processed outside your country of residence.

How long we keep it

China Checkup keeps initial enquiry details and related correspondence for one year after the last communication, whether or not a booking is made. We then delete the copies we control in Formspree and our email account, unless a longer period is required by law. A checkup provider's own records are governed by its privacy practices.

Your choices

You can ask us about the details we hold, request a correction, or request deletion of our copy by emailing mikeparker02141@gmail.com. You may also contact us before submitting the form if you have questions about sharing your details with a provider.
